Treasure Box Your Loved One’s Information Sheet In order for your treasure box to be completed by our art department, we would ask you to provide the following information. We realize these questions may take you to some difficult places in your grief journey; our prayer is for you to do this activity of your grief walk trusting that healing will continue to come to you and those you love. Your authenticity and vulnerability will be honored with sensitivity as we create this treasure box. 1. The name of your loved one. Any nickname ? 2. Birthdate, age of loved one at their death, male or female, other significant dates you want us to know. 3. Your age, male or female, relationship to your loved one, other significant information you want us to know. 4. Please give us 3-5 words you would use to best characterize your loved one. Or give us 3-5 words the recipient, if other than yourself, would use to characterize your loved one. 5. One item, theme, "representation of" or common interest that may be only understood between the receiver of the treasure box and your loved one. 6. Your loved one’s likes, dislikes, and favorites. Possibilities might include (but not limited to) place, activity, sport, phrase, food, art, color, hobby, symbol, dreams, life goal, styles of music, clothing, décor. 7. Would you like anything written on the bottom of the box? Possibilities might include (but are not limited to) a quote, date, brief message, to/from.
